The Home and Community-Based Services Traumatic Brain Injury Medicaid waiver program provides supports and services to people who have survived a traumatic brain injury and want to continue living in their homes. 

Under the TBI waiver, AIM provides Service Coordination, Community Integration Counseling, Positive Behavioral Intervention and Supports, and Independent Living Skills Training. Our service coordinators can also arrange a wide array of other available services for participants in this program. 

AIM's TBI Program serves Allegany, Broome, Chemung, Chenango, Schuyler, Steuben, Tioga and Tompkins counties.

Eligibility

  • Have a diagnosis of TBI or a related diagnosis
  • Be eligible for nursing-home level of care as determined by a Patient Review Instrument (PRI) and SCREEN
  • Be a Medicaid recipient
  • Be 18-64 years old
  • Choose to live in the community rather than a nursing facility
  • Have or find a living arrangement that meets the individual’s needs
  • Be able to be served with the funds and services available under the HCBS/TBI waiver and New York State Medicaid State Plan
  • Not participate in another HCBS waiver.
