Nursing Home Advocacy and Transitions
We advocate for nursing home residents, help them transition to the community and coordinate the services they need to live independently.

Consumer-Directed Home Care
Our home care programs allow seniors and people with disabilities to choose, train and supervise personal assistants to help them live independently.

Assistive Technology
Assistive equipment and technology loans, from wheelchairs and ramps to communication devices. Delivery and device demonstrations are available.

OPWDD Supports and Services
Service Access, Self-Direction, Community Habilitation, and Family Support Services for people with developmental disabilities and their families.

Service Coordination
AIM coordinates services for people in the Nursing Home Transition & Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waiver Programs.

Health Home
Care management for Medicaid recipients with serious and complex physical health conditions, mental health conditions, and substance use disorders.

NY Connects
Information, options counseling, and referral to a wide array of long-term services and supports to help seniors and people with disabilities remain at home.

Supported Employment
We provide job skills training, job placements and job coaching to people with all types of disabilities. AIM also operates AJ’s Deli in conjunction with this program.

Supportive Housing
Housing for seniors, veterans, and people with disabilities who are facing homelessness, with support services available to residents.

Insurance Navigation
AIM’s navigators provide free enrollment assistance for the New York State of Health insurance marketplace.


Veterans Services
AIM provides education, advocacy, information and referral to help military veterans achieve their goals of independence.

Peer Services
Peer mentoring for people with disabilities entering the workforce, people transitioning out of nursing homes into the community, and those in recovery from addiction.

Community Nutrition Program
The Community Nutrition Program provides food and basic goods to people in need up to twice a month, free of charge. Both pick-up and delivery are available.

SNAP Outreach Program
Eligibility screening, as well as application and recertification assistance for the Supplemental Nutrition Assistance Program (SNAP).
