AIM is seeking people to work as peer mentors for the Open Doors program, which helps residents of long-term care facilities transition into the community.

Peer mentors typically have disabilities and use home- and community-based services to manage their healthcare needs. Many have lived in an institutional setting at one time and have personal experience with the transition process.

Family members of people who have received long-term care in facilities or home- and community-based services are also needed to support the loved ones of those who are transitioning.

People who have received services through the New York State Office for People with Developmental Disabilities (OPWDD) are also needed to work with individuals transitioning from an OPWDD residential setting.

Veterans are sought to work with service-connected participants in the Open Doors program.

This is a paid position. Peer mentors typically work less than five hours per month. An eight-hour training session is provided. Right now, all peer mentoring is being provided by phone or video chat due to COVID-19 restrictions.
