The Long-Term Care Ombudsman Program advocates for residents of long-term care facilities. Ombudsmen help residents understand their rights; address issues that impact health, safety and quality of life; and investigate complaints.

Open Doors

The Open Doors program reaches out to nursing home residents who are Medicaid recipients and want to explore transitioning back to the community. We help people move into their own residence and arrange home- and community-based services so they can live independently.

Olmstead Housing Subsidy

This program helps Medicaid recipients move out of nursing homes and find safe, accessible housing. Funding is available to help people move into their new apartments, and participants pay 30% of income toward rent. Our specialist helps with the entire process.

Nursing Home Transition and Diversion (NHTD)

AIM coordinates a wide array of services for participants in the NHTD Medicaid waiver program. We also provide services directly, including Community Integration Counseling, Positive Behavioral Interventions and Supports, and Independent Living Skills Training.
