AIM operates the Open Doors Transition Center Project/Peer Outreach and Referral Program in Chemung, Schuyler, Steuben and Allegany counties. This program helps nursing home residents who are Medicaid recipients transition into the community.
The Peer Outreach and Referral Program uses peer mentors who have successfully transitioned from institutional settings to the community, or who have received home- and community-based supports and services which have allowed them to remain independent.
AIM’s peer mentors share their experiences with people who are interested in transitioning. They provide support and encouragement before, during and after the transition.
Our peer mentors have completed formal training in best practices for facilitating transitions.
