AIM provides Health Home care management in Chemung, Schuyler and Steuben counties.
We are a regional Care Management Agency affiliated with the Greater Rochester Health Home Network, which is part of the statewide Health Home program.
This program provides care management to Medicaid recipients with serious and complex physical health conditions, mental health conditions, and substance use disorders.
To be eligible, an individual must be enrolled in Medicaid and have two or more chronic health conditions. This includes a wide range of physical conditions such as cancer, heart disease, diabetes or asthma; as well as substance use disorders and certain mental health conditions.
In some cases, an individual may be eligible with a single qualifying condition, such as HIV/AIDS, a serious mental illness, a serious emotional disturbance, complex trauma in a person under age 21, or sickle cell disease.
Once enrolled in the Health Home program, a Medicaid recipient will have a care manager to set up a comprehensive care plan, and to coordinate the services they need. This may include:
- Connecting to health care providers
- Connecting to mental health and substance abuse treatment programs
- Obtaining needed medications
- Help with finding stable housing
- Connecting to social services, such as food assistance, public benefit programs, and transportation
- Connecting to other resources available in the community
The overall goal is to improve health outcomes for Medicaid recipients and reduce costs, with fewer emergency department visits and hospitalizations.
Joining a Health Home is free. The costs are covered by Medicaid.
