Medicaid Waiver for Traumatic Brain Injury and Nursing Home Transition and Diversion

The Home and Community-Based Services TBI Medicaid Waiver provides supports and services to people who have survived a traumatic brain injury and want to continue living in their homes. Under the TBI Waiver, AIM provides service coordination and independent-living-skills training.

· Substance-abuse programs

· Intensive behavioral programs

· Community-integration counseling

· Home and community support services

· Respite care

· Assistive technology

· Transportation

· Community transition services

Eligibility

· 18 – 64 years old

· Medicaid-eligible

· Medically diagnosed traumatic brain injury 


The Home and Community-Based Services NHTD Medicaid Waiver provides supports and services to people with disabilities and the elderly who wish to continue living in their homes. Under the NHTD Waiver, AIM provides service coordination and independent-living-skills training.

· Environmental modifications

· Community-transition services

· Moving assistance

· Assistive technology

· Community-integration counseling

· Home-delivered meals

· Home and community support services

· Home visits by medical personnel

· Nutritional counseling/education services

· Positive behavioral interventions/supports

· Respiratory therapy

· Respite services

· Structured day program services

· Wellness counseling services

 Eligibility

· 18 – 64 years old with a diagnosed disability

· 65 years old & older with or without a diagnosed disability

· Medicaid eligible
