Chronic Disease Self-Management Program

This free, six-session program enhances one’s skills and ability to manage his/her health and maintain an active and fulfilling lifestyle.

The peer-led, self-management program is for individuals whose lives are impacted by heart disease, arthritis, diabetes, depression, asthma, bronchitis, emphysema and other physical and mental health conditions.

Participants have reported decreases in pain, doctor visits and medication use.

Sessions are 2.5 hours long and are facilitated by trained staff.

The program was developed by Stanford University.
