Speaker to discuss Prime Care Coordination

The featured guest speaker for AIM Independent Living Center’s HOPE Parent Support Group this month will be Megan Griffin, individual and family engagement coordinator for Prime Care Coordination.

The HOPE Parent Support Group will meet from 6-8 p.m. at the Elmira AIM office, 350 W. Church St.

Griffin will discuss the New York State Office for People with Developmental Disabilities’ transition to the Health Home Care Management model and the formation of regional Care Coordination Organizations - including Prime Care Coordination - and what the changes mean for people with developmental disabilities and their families. 

She will explain Care Managers, Life Plans, and choices and services and providers.

AIM partners with Prime Care Coordination to provide care management.
